Important information
Educational tool, not medical advice. MyAllergyPlan and the plans it generates are educational. They are produced automatically from the answers you provide and have not been reviewed by a clinician. They do not diagnose any condition, do not prescribe or dose any medication, and do not replace evaluation by a licensed clinician. Decisions about testing and treatment should be made with a clinician who knows your history.
Not an emergency service. This website cannot help you in an emergency. If you or someone with you has trouble breathing, throat or tongue swelling, faintness, widespread hives with vomiting, or any symptoms of anaphylaxis, use epinephrine if it is available and call 911 or your local emergency number immediately. Do not wait for a plan, a call, or a message.
No clinician-patient relationship. Using this website, completing a plan, or receiving a call from a care coordinator does not create a clinician-patient relationship. That relationship begins only when you complete a visit with a clinician of Curex Medical Services, PA, and agree to its terms of service and telehealth consent.
Treatment options and devices. Where this site describes categories of treatment or types of epinephrine devices, it does so to help you prepare questions for a clinician. It does not recommend any treatment or device, does not state that any treatment is appropriate for you, and the device illustrations are generic and do not depict any specific product. Whether any option is right for you can only be determined by a licensed clinician after evaluation.
Clinical care. Medical services are provided by Curex Medical Services, PA, an independent professional medical practice, and by its licensed clinicians. MyAllergyPlan is operated by Curex, Inc., which provides technology, marketing, and administrative services and does not practice medicine. Availability of visits and clinician licensure vary by state.
Program sponsorship. Some parts of this website — identified on the page — may be supported by a program sponsor. Where that is the case, the sponsor's support is disclosed on that page. Sponsors do not provide medical care, do not receive your personal information, and have no role in the content of your plan or in any clinician's recommendations. Curex does not receive any payment from any sponsor based on the number of people who use this website, receive a plan, complete a visit, or receive any treatment.
Your information and consent. By requesting a plan you agree that Curex may contact you by email, and — if you provide a mobile number and opt in — by text message, about your plan and about allergy care from Curex. Message and data rates may apply; reply STOP to opt out at any time. Your information is handled under the Curex Privacy Policy and Terms of Service, and once you become a patient, under HIPAA and the Curex Medical Notice of Privacy Practices. We do not sell your personal information and do not share it with any program sponsor.
Costs. The plan is free. Clinical visits and laboratory tests are billed to your insurance where covered; your care coordinator will give you an estimate of your out-of-pocket cost before you commit to a visit. Self-pay prices shown are estimates and may vary by state and service.
Accuracy. Content on this site is based on published clinical guidelines and is reviewed by Curex Medical's clinical leadership, but medicine changes and your circumstances are unique. Statistics cited are drawn from published research (including FARE and CDC/NCHS prevalence estimates) and are provided for general context.
